You’ll find our offices near the lakefront in Lincoln Park / Lakeview in the AMITA Health Professional Office Building at the corner of Sheridan and Diversey.

How to Find Us

To Reach Us By CTA Bus:

The #151 (Sheridan), #156 (LaSalle) or #76 (Diversey) all stop in front of the medical center. The #77 (Belmont) stops in front of St. Joseph Hospital (a short distance away).

To Reach Us By Rapid Transit Train:

Red Line to Belmont stop and #77 Bus east to Hospital as above or Brown Line to Diversey Stop and #76 bus east to Sheridan

To Reach Us By Auto:

90/94: exit at Diversey, then go east to Sheridan. 
Lake Shore Drive from north: exit at Belmont; turn left (south) on Sheridan; go 6 blocks to Diversey.
Lake Shore Drive from south: exit at Fullerton; turn right (north) on Cannon Drive; go to Diversey.

Parking

Valet parking is available in the driveway located off Diversey, west of Sheridan. The cost is $8.00, cash only, with office validation.
